TLS offers a variety of supports to individuals with Developmental Disabilities and/or Traumatic Brain Injuries:
Service Coordination
Transitional Living Services provides Medicaid Service Coordination (MSC) and encourages families or individuals who need this service to apply by calling Sharon Moore at TLS (315-218-0825). The MSC Team at Transitional Living Services includes staff who are fluent in Spanish and American Sign Language. Several Service Coordinators have over 20 years experience providing services. Areas of expertise include support to adults, children and people with multiple disabilities. People receiving MSC live in a variety of settings in Onondaga County, including their own home, apartments, IRAs, certified Family Care and living with parents, family members and friends. Please check the DD/TBI Outreach page on our website for other services available at TLS. · Our experienced Service Coordinators assist individuals to design and access the services they need to live their lives the way they choose. · The Service Coordination team at TLS works to assure that supports are available and that resource people are working together to help individuals live successfully in the community. · TLS Service Coordinators use a creative approach to advocate for and help people achieve their life goals.
- I.S.S. (Individualized Support Services) is a funding stream that provides financial assistance to help people live independently in the community of their choice.
- I.S.S. funding can help pay for such things as home furnishings, security deposits and rental stipends.
