Mental Health Community Outreach Programs
Mental Health Community Outreach Programs provide individuals diagnosed with mental health issues the opportunity to establish and maintain a variety of supports flexible enough to meet their changing needs. Services are provided with a person-centered philosophy that encourages the individual’s active participation in determining their goals and the strategies developed to achieve them. MH Community Outreach Programs include:
- Supported Housing through our Independent Housing Program (IHP) assists individuals with establishing safe, affordable permanent housing by:
- assisting with the search for an apartment, moving expenses, security deposits and rental assistance when appropriate
- securing basic furnishings and household supplies
- providing advocacy and support to maintain their independent living situation.
- Bridge Services at TLS works with individuals receiving other TLS assistance to:
- provide community-based supports to adults who are struggling to manage or make progress in their pursuit of the quality of life they desire, and seek greater levels of wellness, independence, and personal satisfaction and success in their lives.
- support people in the development of their recovery goals focusing on physical health, independence and coordination/instruction related to accessing transportation.
- TLS’s Assisted Competitive Employment (ACE) and Supported Employment programs provide eligible individuals with a variety of employment-related supports including:
- resume development, interviewing skills and job search assistance
- on and off-site job coaching
- ongoing support and advocacy designed to assist individuals with maintaining their employment
- (Supported Employment also provides these services to individuals with eligible Developmental Disabilities.)
- Provisions Bakery in Armory Square provides individuals diagnosed with mental health issues an opportunity to:
- develop employment-related skills which will transfer to a variety of competitive employment fields
- learn about appropriate workplace attire and behavior and develop the social skills necessary to work with coworkers
- establish a successful work history that will improve access to employment opportunities in the community.
- The Supported Education program at TLS provides support designed to:
- help individuals with mental health issues to identify and achieve their educational goals
- determine which school or course of study will best suit the students’ goals
- assist with the application process, applications for financial aid, and course registration
- advocate for academic accommodations when appropriate
- develop time management and study skills.
- provide continuing support throughout a student's tenure in school
- Community Support Services (CSS) assist individuals with:
- developing friendships and getting involved in community activities
- identifying the barriers that keep them from being involved in activities and developing strategies for overcoming those barriers
- budgeting, learning about transportation resources and identifying opportunities for social involvement.
