Residential - Mental Health
TLS’ residential services enhance community-living opportunities for adults who are involved with the mental health system.
TLS provides rehabilitation services in community settings to enhance living opportunities for adults who need assistance in developing community living skills and supportive networks of their choice. Participation in TLS programs is voluntary and based on individualized service plans developed by the consumer, with regard to the following services:
- Assertiveness/Self-Advocacy
- Community Integration Services/Resource
- Daily Living Skills Training
- Health Services
- Medication Management and Training
- Parent Training
- Rehabilitation Counseling
- Skill Development Services
- Socialization
- Substance Abuse Services
- Symptom Management
Individuals are encouraged and assisted to build a network of resources and support people who can help them to achieve a sense of well being and belonging. The amount of staff support provided varies according to individual need. In most cases, staff are either on-site or available (through a system of radio pagers) on a 24-hour basis.
Residential Services Funded by NYS Office of Mental Health (OMH):
Congregate - participants receive assistance from staff members who are present on-site on a 24-hour basis whenever participants are at the residence. Congregate programs are offered in home-like or apartment settings.
Apartment Program - participants receive assistance from staff members who support participants with face-to-face visits from three to seven days per week and are on call 24-hours a day, seven days a week. Participants typically live in their own apartments or with a roommate. Transitional Living Services offers an opportunity for consumers to "turn-key" their apartments. At the time of discharge, an individual may choose to assume the lease and thereby remain in their chosen neighborhood. TLS staff would then secure a new apartment for an individual and/or individuals on intake, or possibly certify their existing apartment.
