Community Based - Mental Retardation/Developmental Disability
The Individualized Outreach Program (IOP) assists adults who live in Onondaga County and are labeled with a developmental disability or Traumatic Brain Injury (TBI) as defined by the New York State Office of Mental Retardation & Developmental Disabilities (OMRDD) and/or the Department of Health (DOH). Priority is given to individuals leaving TLS certified OMRDD residential programs. The IOP also gives special emphasis to serving Medicaid Waiver participants looking for a choice in case management services. In addition, individuals being referred to residential programs certified by OMRDD could be offered assistance if it were deemed an appropriate alternative by their support network.
The Individualized Outreach Program (IOP) Service Coordinators provide the following:
- Bridge services after TLS residential programs - This typically includes monthly support and training to assist individuals with maintaining their independent living arrangements and skills. The IOP team members may also assist the residential team in developing independent living options and services to ensure that an individual's needs and preferences are being met. Referrals are made at least three months prior to an expected transition date and the residential team typically continues to support the individual for up to three (3) months after the transition.
- Service Coordination (MR/DD) - TLS' Service Coordination Team works to insure that supports are available to help individuals live successfully in the community. Practical assistance is
offered to support each person in the living situation of their choice, as well as, advocacy and accessing of other necessary supports.
- Individualized Support Services (MR/DD) - This is a funding source to assist adults with living more independently. This may include financial assistance with security deposits, rental stipends, home furnishings, setting up phone service, and other community supports.
- New York State Department of Health Waiver Service Coordination (TBI) - For individuals living in the community or in other residential settings that need a variety of services. These include the development of individualized plans, living arrangements, supports and specific services which are needed.
Consistent with TLS philosophy, it is intention of the Individualized Outreach Program to serve people on an individual basis according to their choices, desires, and needs.
